Oak Glen Middle School - Band Instrument Usage Agreement

Dear Parents:

We are pleased to provide your child with a school-owned instrument for his/her use during the school year. At the end of the school year, the instrument is to be returned to the school. It can then be reissued for the following year, when a new agreement has been completed.

The instrument is issued in good working condition. Any damage to the instrument is the responsibility of the parents to repair. (Unless it is normal usage wear and tear.)

Hancock County Schools is not responsible for instruments left on the school bus, in student lockers, or in unsecured facilities.

In consideration of Oak Glen Middle School releasing an instrument to my child, I agree to take full responsibility for the care of the instrument described above, including loss or damage to the instrument beyond normal wear and tear for the school year. I understand that I will be responsible to pay for any damage to the instrument. I also understand that the instrument must be returned on the last day of school. *
